When you get a vaccination or a negative test result, you can ask your health care provider to send a request to Cert.Health for a certificate as evidence.

Any Cert.health registered Verifier can scan your QR code, and instantly get validation of your vaccination or a negative test result.

Your Healthcare provider will give you a QR Code, that you can either print out, store in the Cert.health app, or another other digital wallet that can store a QR code.

Where will I get my certificate?
You will receive your QR code certificate verification link (via email from Cert.Health) . You can store the QR code in our app (download link) or in any wallet app that is compatible with QR codes The QR Code does not contain any personal information, just a link that a verifier can use to verify your certificate.
Is there a fee to use your service?
If you choose to store your certificate in our online digital wallet the service is free. If you download the certificate for offline use, there is a one time fee of $10.
What information will the Verifier see?
The certificate will show your name, DOB, the date you received the vaccine, the name of the healthcare provider that administered it, the type of vaccine, and the vaccine manufacturer.
Does my health information remain private?

Name, DOB, the date you received the vaccine, the name of the healthcare provider that administered it, the type of vaccine, and the vaccine manufacturer is the only data we will have access. We only use that information to generate and validate your vaccination certificate.  View our Privacy Policy for more details.

Do I need to sign a consent form to comply with HIPPA?

Healthcare providers require all patients to sign a patient consent form to comply with HIPPA regulations when disclosing any healthcare data to a third party, including immunization records and lab results (e.g. a negative Covid-19 test).  Download the consent form here and bring it to the healthcare provider performing the immunization or lab test to sign.

Who will be able to validate my certificate?
You will present your QR at any venue, event, or location that requires you to be vaccinated to enter. The verifier must be registered and approved in the Cert.Health system. All verifiers undergo extensive identity validation. They will scan the code and verify that your vaccination is valid and up to date.

When does the certificate expire?
The certificate expires whenever the healthcare provider specifies, typically 1 year. You will likely need to receive booster shots and a yearly vaccine to keep it up to date. The rules around this are still evolving and likely will continue to do so for some time.
Will I be able to update my certificate with any booster shots?
Yes. When you go in to receive your vaccine booster, the healthcare provider will generate a new certificate and QR Code verification link.
Does it require me to download a new app? Is the app free?
We do have a Cert.Health app you can download to store your QR Code. The app is free to download and use.

Your certificate can be stored within our proprietary app (free to download) or within any wallet app that supports QR codes.
